Missed Quarterly Inspections
NFPA 25 mandates quarterly visual inspections of sprinkler systems. Facilities often complete the physical inspection but fail to produce dated, signed records when the surveyor asks for them.

No ITM Records on File
The standard requires records to be maintained on-site and made available to the AHJ on request. Missing, incomplete, or undated ITM records are among the most common citations issued.

Corroded or Obstructed Heads
Sprinkler heads require periodic inspection for corrosion, loading, and obstruction. Heads showing any of these conditions must be replaced — and the replacement documented with the original finding.

Failed or Undocumented Flow Tests
Annual main drain flow tests and five-year obstruction investigations are required. Many facilities either skip these entirely or lack the documentation showing the test was conducted within the required window.

Fire Pump Test Records Missing
If you have a fire pump, weekly churn tests and annual performance tests are required under NFPA 25 (and NFPA 20). The overlap between standards means gaps in one compound into the other.
